To minimize the risk of exposure and keep our patients and staff healthy please advise the office if any of the following applies to your or members of your household within 14 days of your appointment.

Fever
Chills
Cough
Runny or stuffy nose
Sore throat
Shortness of breath
Headache
Unusual fatigue
Body aches
Loss of taste or smell
Nausea
Vomiting
Diarrhea
Traveled outside Washington State
Had exposure to a n individual who tested positive to COVID-19

Reminder: Wear a cloth face covering to your appointment and call when you arrive at the office. This will ensure that we are taking all the necessary steps to minimize contact and exposure.
